H A P P Y   H O L I D A Y S !

Welcome to The CareLink Foundation! We are a 501(c)3 non-profit organization. Our mission is to provide quality health care for medically under-served or under-insured children whose family's

income level is at or below 200% of the federal poverty level in DuPage County, Illinois. Services

are provided by a staff of volunteer physicians, nurse practitioners, nurses and non-clinical workers

who believe that each child is special and should have an opportunity to grow up healthy. Area

churches, schools and park districts donate space to hold clinics.

Working hand in hand as a community the following services are offered:

School physicals and necessary vaccinations for children to be able to attend school.

• Sports physicals in order for children to participate in after school activities.

• Provide new car seats for infants and toddlers and educate parents on the proper use and installation.

• Refer medically challenged children to pediatric sub-specialists.

• Arrange health care interpreters to accompany families to specialists and reimburse for such services.

• Provide taxi rides to specialists if family does not have transportation.

A bi-lingual certified child car seat passenger technician provides instruction to parents on the proper installation and use of car seats. Safety classes are taught throughout DuPage County but the majority of these classes are held in the Health and Education Center in West Chicago.

Funding for CareLink is provided through grants, private contributions and annual
fundraising events.
